Darkroom:

• Verify Accuracy of Developer Temperature is it unstable or set too high?
• Verify Accuracy of Developer was it Properly mixed or Off with too little water.
• Verify that Developer Replenishment Rates are Properly Set and not set too High.
• Verify If Proper Amount of Starter was added to the Developer.
• Verify Incoming Water Temperature that it is cold and not too Hot.
• Verify that Incoming Water Valve is On and Water is Circulating Properly.
• Verify Films Quality and Proper Storage, Check Expiration Date.
• Verify that Film has not been exposed to Chemical Fog or Excessive Heat.
• Verify that Film has not been exposed to Scatter Radiation or Light.
• Verify Integrity of Film, Cassettes & Screens.

X-Ray:

• Verify Proper Exposure Factors. (kVp/ Ma/Timer set too High/Over Exposure?)
• Verify Proper Focal Distance. (Too Short Focal Distance?)
• Verify Calibration of System. (kVp/Ma/Timer Accurate?)
• Verify if Power deviates During the Day or while Making Exposures.
• Verify Accuracy and Proper Positioning of Patient.
• Verify Accuracy of AEC System and Incorrect Centering of Patient to Chamber.
